What documents/information will I need to enroll my child?
- Proof of Residence within our school boundaries Verification is done by: Current Electric or gas bill (not water), rental or escrow papers
- Verification of Age: students must be age 5 on or before December 2nd of the school year they are enrolling in Official Birth Certificate (not the hospital document) or a Passport
- Current Immunization Records -see guidelines below:
Polio– 3 doses (last dose must be on or after 4th birthday, or 4th dose is required. Last 2 doses 2 months apart) |
DTP/DTaP– 4 Doses DT/Td (last dose must be on or after 4th birthday, or 5th dose is required. Last 2 doses must be 6 months apart or counts as 1 shot) |
MMR Vaccine– 2 doses or 1 MMR and 1 Measles containing vaccine meets requirement (both doses must be given on or after 1st birthday) |
Varicella (Chicken Pox) 1 Dose (dose must be given after 1st birthday or disease to be documented by health care provider) |
Hepatitis B—3 doses (we will accept registration if the series is in progress) HBV 2nd dose 1-2 months after 1st dose, 3rd dose 2-6 months after 2nd dose, at least 4 months between 1st and 3rd doses |
Physical Examination: Although a Report of Health exam is not required for Kindergarten Enrollment, it is required by December of a student’s first grade year. An exam prior to entering Kindergarten will meet this requirement. This exam can be completed no more than six months prior to kindergarten enrollment.
Oral Health Exam: California Education Code now requires that your child have an oral health assessment (dental check up) by May 31st of his/her Kindergarten year. Forms are available in the registration packet.

When will enrollment begin?
Kindergarten/New student registration day will be on Tuesday, April 13th 2010 at 10:00am. There will be an informational meeting for parents only. Parents/Guardians can pick up enrollment packets anytime during office hours 8:00-4:00pm M-F after March 9th, 2010.
What is “Choice Transfer?”
The initial application period for a Choice Transfer will begin on February 23rd, 2010. For information please call the district CWA office at 714 985-8670. PLEASE NOTE: Siblings of students currently on a Choice Transfer to Brookhaven are not automatically approved. They must apply for a transfer.
